Verification of Residency
Documents Required to Verify Residency
All families are required to provide the following documents to verify residency (or to re-verify residency after a move within Mill Valley):
1. ONE of the following documents showing the name and address of the parent, guardian, or caregiver within the District:
- Copy of the recorded grant deed
- Copy of the most recent mortgage statement
- A fully executed copy of the lease or rental agreement, PLUS Lessor/Lessee Supplemental Form completed by the landlord
2. ONE of the following documents showing the name and address of the parent, guardian, or caregiver within the District.
- Utility Bill (Must be dated within thirty (30) days of school registration documents)
- PG&E
- Marin Municipal Water District
- Mill Valley Refuse/Tamalpais Community Services District/or other local refuse agency serving residences within district boundaries
- Comcast (cable TV)/DirecTV

Information About Residency Verification:
Residency: District Residency Administrative Regulation 5111.1
- All families are required to provide 2 proofs of residency at registration;
- Families must notify the District/School when residency changes;
- All families are required to complete a new Declaration of Residency and provide 2 proofs of residency if they move within the District;
- Students that move and reside outside of the Mill Valley School District boundaries must immediately register in the District in which they reside.
- All families that rent or lease homes, and their landlords, will be asked to complete the Lessor/Lessee Supplemental Form.
